i just moived from methadone, cant spell, to fentanyl

i just moived from methadone, cant spell,  to fentanyl,   i was taking 120mil through out the day,   now my doc put me on  Fentanyl  25 mcg/hr patch.

will that work the same or better?

     I'm not sure but if I understand, you're asking if 25mcg fentanyl is equal to 120ml methadone. There is an opiod comparison chart on the site if you search for it. I'm not sure it would be equal though due to the longer half life of methadone. You or someone more knowledgable than me will have to look it up.

     If you are asking for experience in switching from methadone to fentanyl, be patient, it may take some time for someone with that experience to read and answer your post.

     If you are playing around with these two meds, DON'T! they are both very powerful and should not be used except under MD supervision!

Sounds low...

if you do withdraw, you won't notice anything for a good 24-48 hours after your last methadone dose. Good luck...

I think you might be getting underdosed...Theres alot more to be taken into consideration that "potency per mg;" half-life being one of them...

The conversion chart I have only addresses......

Oral methadone in mg/day. In order to convert that to Fentanyl I would need to know your former dosage in the weight (mg/d) context, not the volume (ml/d) context. If you would like, p.m. me with more info. I am not a doctor, just someone who is Rxed Fentanyl. I know Zero about methadone except that it has a very long 1/2 life which certainly complicates matters. Have you asked your Doc?

In my opinion, depending on how long you have been taking Methadone (chronic or acute), you are being WAY underdosed.  I am assuming that 1ml equals 1mg, so that is 120mg of Methadone.

Go to this narcotic dosage convertor.   http://www.globalrph.com/narcotic.cgi    Plug in 120mg of chronic Methadone and you get 1.5ml/day of Fentanyl, a 100mic and a 50mic patch.

You will certainly know within a few days of stopping the Methadone.  Everyones' body is different so seek your doctor's permission before changing your dose.

Haha...thinkin the same thing, especially most people pronounce it (FET-NULL)...

 

But, on a serious note, methadone does have a longer half-life as a whole, but as an analgesic, it's not really even close to as long (hence, why opiate management is only required once daily, wheras PM requires multiple doses throughout the day)  Methadone actually is a great painkiller though.. but i'd have to say for chronic pain, fentanyl is the best and most convenient. Definately in regards to taking the meds, and not on the wallet Money mouth

Switching from methadone to fentanyl will not be a problem. If anything, you will notice your life getting easier as you won't have to remember to take all those pills. Instead you may have some trouble with your patch staying on or, having trouble getting the adhesive off of your arm. I would recommend taking the duralgesic patch simply because you can see the medication and sometimes it does last longer than 72 hours. I have switched from almost everything to fentanyl at one time or another. I had never heard of it when my doctor recommended it. I had absolutely no idea what it was all about. I had been taking a combination of some other meds and wanted to be on those desperately. I put on the patch and laid down to watch TV. After about an hour, I noticed that I hadn't thought about my other meds and quite frankly didn't care if I had them. I remember thinking that if my preferred meds were on the table across the room, I wouldn't have gotten up to get them. I took fentanyl and seroquel very successfully for a good period of time. The withdrawals were not as severe as methadone withdrawals either. I did have to take suboxone for a few days to get off but, it wasn't a life and death feeling like I have had so many other times.

One of the most important factors in switching from any medication to fentanyl is the impact the pills have on your kidneys and liver, even your stomach. These organs may have been comprimised due to your condition. In taking the necessary amount of pills to manage your pain, it may have an adverse effect on your organs. The patch does get processed by the same organs but it is much less dramatic of an effect. I have taken fentanyl off and on for quite some time and I always go back to it. I would rather deal with the patch than keeping track of all those pills.

Uhh, Thanks, but I didn't ask the question. I've been on fentynal for years and will never come off. I'm a patient for life unfortunately. I am very well aware that switching from any med to any med can be done. The OP was asking for specific equopotency(sp) and I told them I wasn't sure of the conversion to be patient and someone may be able to help.

All those pills? I thought methadone is usually only dosed once daily. But I've never taken it so thats why I told the OP to be patient and wait for someone who had to answer.

...you breathe in most major metro areas is just as %@$$*## the liver as APAP if it's taken at a proper dose.

P.S. Few things in life are as bad as high dose methadone withdrawal.
