percocets to oxycontin dosage?

I have been taking 6 10/325 percocets a day. My doctor put me on 10mg oxycontin today every 12hrs with only 4 x 10/325 percocets a day. Why would he change to such a small dosage instead of to 30mg every 12hr with only 2 10/325's a day available for BT pain. There is no way the 10mg every 12hrs is going to help when i've been taking a 10/325 every 4 hrs. What do you guys think. I did get a script for 180 percocets like 10 days ago from him so maybe he wants me to use up all the percocets then increase the dosage next month when i see him 30 days from now. Please give me you input. I did ask him to switch to a long acting like he talked about last time and he said that taking PRN (which i need every 4 hrs and isn't prn any how) is supposed to build tolerance slower. I would rather not have to take 10 million pills. What should I tell him to get proper dosage of oxycontin. I even told him I did not want to go to oxycontin because of the stigma with it and try something different. He said the oxycodone I am taking now works so why mess with it and that oxycontin is the same drug. I was able to get the brand name through my insurance so thats cool because brand names always work better. My co-pay was 25$ for 60 10mg oxycontins. I just wanted to share with you guys my experience and get some advise to get the proper baseline dosage next time I see him. I am not sure the 4 10mg percocets a day is going to be enough I think the baseline med is going to be to low to even make it through the day with what he gave me. I am not a drug seeker just want to through that out there. I am not looking to get high just trying to not suffer in pain.  Wish me luck and please give me so help here. Thank all in advance for your responses. Dr. Lois I know your on here I would appreciate your response here. I didn't really know what to tell him and was afraid to tell him that the dosage was going to be to small. I going to try it out and try to make it I just personally think that at least 20mg every 12hrs make more sense. I wish everyone a pain free day.

The reason he gave you the

The reason he gave you the lower LA med with some breakthrough is so you build tolerance slower as you said. He obviously feels that you dont need a constant high dose of opiates but feels you need a lower base line with the option of taking some immediate release if the need arises. I am sure everyone would rather just take a higher dosage of extended release med so you are never in any pain and dont have to take more pills just the 2 oxycontins but your tolerance will rise too fast. If the 10mg really isn't cutting it then explain to your doctor that your running through all your breakthrough meds and think you should move to the new 15mg dosage. The way I understand it is that you should not be using all of your breakthrough meds and if you are then your extended release needs to be raised. You are right in not contradicting the dose with the doctor before you tried it though because doctors dont take kindly to you telling them what works if you havn't tried it yet. So try your new regimen and if it doesn't give the relief you think it should then on your next refill appointment mention it to your doctor or if the pain is really unbearable give him a call and ask what he thinks you should do and he will probably raise the dose. I would also look into getting TID dosing compared the the BID you are because I find that the extended release oxycodone to not last the full 12 hours although not all doctors are willing to do this. Best of luck

Hopefully he has you

Hopefully he has you scheduled for a follow-up in 1-2 weeks to see how the change in dosage is doing.  Lowering tolerance is always a good thing to try, and with the long acting medication always on board you may not need to play catch-up with high and frequent doses of short acting meds.

If I had been precsribing 60mg a day of percocet 10/325, I would probably have given you 40mg a day of long acting oxycontin (rather than the 20mg you got) and 60 percocet for the month, and encouraged you to make them last at least a month, but that is just me, not gospel.  I might also have tried something out of the codone family to take advantage of any incomplete cross tolerance.   Either way, your average daily dose of oxycodone would stay at 60mg.

My next appointment is not

My next appointment is not until one month. I'm going to try to see how long I can last and the call him and ask what I should do. Maybe I should just call and schedule an earlier appointment. What do you think I should do. I didn't want to challenge his judgement. Is it really true that taking it every 4hrs is going to build tolerance slower than the 12hr dosing? I don't think this can  be true. From what i understand your supposed to switch to long acting if you need that many prn dosages. If it were only 2-4 times a day the prn would be better I guess but as often as I need it the longer preparations make More sense to me.

Well I went to the doctor

Well I went to the doctor today I could wait anymore and he increased my doasage to 20mg oxycontin once in morning and again at night with as many 10mg percocets I need. I still have like 96 left from the refill on the 27th for 180. so I get up to 6 per day. I think I made the doctor feel guilty for low balling me with the 10mg oxys. Well I should have enough meds for relief so everything worked out. Just wanted to keep everyone posted. Thank you for your posts and help.
