Update on Pain ManagementWell I just got back form the PM doctor. I only saw the PA this time. Next time I will need to see the MD. This is all for Chronic Pain follow a femur fracture and rodding in 2004. I still have the rod in my leg. The doctor increase me dosage of the oxycontin from 20mg every 12hrs to 20mg every 8 hrs. He has also given me 10/325 percocets every 6hrs as needed. I am hopping to eliminate the need to take the percocets or at least cut down to 1-2 a day max. I'm not sure if the oxycontin dosage is going to be enough. I told the doctor like one of you suggested that I would like to continue to increase the oxycontin till my pain level is under a 6 without the percocets. I don't think this is unreasonable. He didn't really want to increase the oxycontin. For some reason he wants to keep that dose as low as possible. I think he knows I will be treated with opiates for the rest of my life and only being 26 he wants to minimize the tolerance to the medication so that it stays effective for as long as possible. I asked him about fentanyl patches since I was on these before and founf that its better than the roller-coaster effect of the pills. The percocets are very effective at relieving my pain now that I am on oxycontin they work better than before. The baseline med still seams to be to low so he said we can talk about fentanyl with the MD there next visit. I am hoping this level of oxycontin will be enough but I think I will need to go to the 30's next vist or the fentanyl to cut back on the percocets like I would like to. I don't want everyone to know what I take and having to take pill all the time its hard. What dose of fentany would I need? Does anyone have any suggestions? Should I just continue on the oxycontin until it is no longer effective? I appreciate all of your time and suggestions. I love being able to talk about my pain and treatment here on this forum. It is difficult when your family and friends aren't open to listen and look down on you for getting the medication you need to live a fulfilling life. Thank you all for making me feel at home. I know how you feel about family & friends not understanding. My hubby just says, "Why can't you take a Tylenol or Motrin?" "Why do you need these narcotics?" I just tell him that if he ever needs pain meds for some reason (God forbid) I will remind him how he treats/treated me when OTC meds just don't work. I am currently on the Fentanyl patches and I was on the Oxycontin 20's 3 times a day. My doc put me on the 100mcg/hr dose of the patches. I replace them every 3 sometimes 4 days. I no longer need the Percocets b/c I have no more break-thru pain. The patches are a Godsend. No more roller coaster effect. Actually I am the one who told the doc that I wanted off the Percocets. I don't really think I need them being on the 100mcg/hr patches. Have you researched about the patches? Just tell your doc that you really think the patches are what you want to try. He might not start you at the 100's though. There are 25's, 50's, 75's & the 100's. Good luck & keep us posted................KK Wildman
Sam, wildman I am 52 years old and have been taking narcotics since I was 17.First to get high on and then quit for several years.I worked as a furniture mover for several tears and had two herniated disks a year apart from one another.The lorcet 10/650 I was prescribed lasted/worked well for ten years and then quit working.I was taking the maximum dosage due to the tylenol content.My doctor was good enough to give me liver tests every six months which is needed.But the medicine did not work.My doctor switched me to kadian which is a time release morphine.Very expensive!!! I had another injury on the job where I was pushed backward over a steel stair rail so that the back of my head was touching my heels.It was a permanent injury they decided after many many tests.They gave me evry known narcotic exept morphine and none of them worked at all.I now take over 300mg of ms contin per day and it is the "only" medication that actually works for me.I expect to be taking this medicine for the rest of my life.I'm immune to other narcotics.I also have an extremely high tolerance for opiates of all kinds.I still cannot get enough medicine from my pain specialist doctor who is also my primary care doctor.Consider yourself lucky.Don't worry about the narcotics . Worry about the tylenol if you have to worry.By the way ms contin is much cheaper than the stuff they will try to give you if you have insurance like Avinza/kadian etc..Ms Contin is the same exact thing only much less expensive.I suggest you discuss this with your doctor.He can prescribe you an equivalent dose of ms contin and I promise you it will be much more healthy for you.
