help..any suggestions on a good breaktrough pain med??

hi, im kevin from north carolina. i have fibromyalgia and ddd. i am currently taking 60mg of methadone for major pain and 30mg of oxyir for break pain.  the oxy isnt getting it anymore nor has it for awhile.  any suggestions.

I dont know what is equivalent but as far as potent immediate release meds for breakthrough you can go with Dilaudid 8mg or Morphine sulfate immediate release 30mg. You could also look into the actiq fentanyl lollipops.

No problem. Dilaudid 8mg would be my first choice behind the OxyIR 30mg as it is the closest to oxycodone in relief for me although my calculations show that the 8mg is only equal to 18mg of oxycodone so you might want to ask for a couple extra pills than you are getting for the 30mg roxicodone. The morphine makes me too tired and the fentanyl makes me nauseous.

question about dilaudid

hello again. is the dilaudid expensive?  have no insurance due to loss of job due to fibro and chronic pain

Break thru pain med's~ Fibromyalgia

 Ive been noticing more and more on TV about the drug called Lyrica now being the one & only perscriebed med for treatment and pain of Fibro. Ive had fibro for 19 yrs now.

I tried lyrica for a bit, but made me so dizzy & sleepy i couldnt function at work, but it did help with the breakthru pain.

maybe check with your doc. about Lyrica????

It has been shown to be addicting

Just like any other opiate it can be addicting! But when taken as prescribed, it is highly effective for this type of pain. this drug has been mentioned before so what I got from it was it might take about 1 week for concentrations to be effective to the point of true relief. Good Luck!!

Im curious to where you came up with that calculation?  not that I disagree in that it feels like 18mg, but it more like equivalent to 40mg or oxycodone.  To be honest to the Original Poster, when you take that much methadone not much works for BT pain.  You may want to talk with the doc about increasing the Methadose, or maybe Fentora or Actiq which is about the only thing that might break thru.  Good luck.

I did the calculation based

I did the calculation based on the opiate equivalence chart on this website. It is very possible my math was wrong though so feel free to double check. I did the calculation this way. the chart said that 2mg hydromorphone=4.5mg oxycodone=10mg oral morphine. So if 4.5mg of oxycodone equals 2mg hydromorphone then multiply the 4.5 by four because 8mg of hydromorphone is 4 times the 2mg that the chart says is equal to 4.5mg of oxycodone. This gives you 4.5x4=18mg. The thing is that if i do the calculation based on the equivalence to 30mg codeine instead of 10mg oral morphine which says 1.5mg oxycodone = .6mg hydromorphone then I come up with an equivalence of 8mg hydromorphone being equal to 19.99mg so I am not sure how consistent those values are on that table because the ratio is changing when they say what is equal to 30mg codeine or 10mg oral morphine and there is no way one of the opiates is getting stronger when you compare it to one but not the other. if anyone can help clear this up please chime in because now I am very confused.

It's hard to say, but with the narcotic converter I use with 30% incomplete cross tolerance I am getting 8mg of dilaudid orally = to almost exactly 30mg of oxycodone.  However we all know it sucks.  What about Panlor as a BT med?

I did not read all the posts but I use the Oxy IR. It used to be the 30mg but I asked my doctor if she would give me the 15mg. She agreed and it ends up the same amount but I really started seriously trying to lower my b/t 's and it is working quite well. I have been able to have some left by the end of the month. But I am sure other's who posted like their b/t pain med's for one reason or the other and you might not like the first one's you are prescribed as I did. The first one's had side effects I did not like.

Oxy Qustion ~ Can you help?

Hi.  I'm new to the forum and you sound knowledgeable about pain meds.  I was in an auto accident and I've been in chronic pain since.  I currently take 60mg oxy twice a day and my Dr insists that he is only allowed to perscribe it (by law) the same dosage in the the a.m. and the p.m. within a 24hr period.

I read another post that said someone was perscribed it 3 times a day.  The extended release is 8-12 hours and by the middle of the day ~ I am in pain again.  We tried percocet as a break through med but that didn't touch it.  I asked for a lower dosage of oxy but he claims he can't legally do it and my insurance won't cover it.

Do you know anything about this?  Thanks, Spunky

 If 30mg OxyIR isnt touching the pain then Panlor will be like taking a flinstone chewable vitamin. Panlor contains Dihydrocodeine,apap and caffeine. Dihydrocodeine is 1-1.5x the strength of Codeine. The OP would have to take like 6.25 Panlor SS tablets to equal the 30mg OxyIR, and with that many pills comes about 4455mg of APAP and 375mg of Caffeine. Hi Spunky!

What part of the country are you in? Just curious because it seems that in some areas the doctors are a lot stricter with prescribing Oxycontin.

I am Rx Oxycontin 60mg x 3 per day, every 8 hours, so I don't know why your doctor is saying he/she can NOT Rx 3 x day by law. A lot of people on Oxycontin are prescribed 3 x per day.

Have you asked your doctor about Roxicodone for breakthrough pain? It is oxycodone, immediate release, with NO Tylenol. I switched last month from Dilaudid to Roxicodone and it works A LOT better for me.

Oxy dosing

Hey Spunky...I don't know what your doc is trying to do by telling you something like that, but it's a bunch of bull.  He just doesn't want to prescribe anything stronger or more often.

When I was on Oxycontin several years ago, my doctor had me on it TID from the first presciption he wrote.  I think he started me at 20 mg TID, with 5 mg Roxicodone for BT.  Anyway, in a little over a years time, I was up to 160 mg (2X80mg) TID and 30 mg Roxicodone for BT PRN.

I guess what I'm saying to you, is that your doctor isn't being straight with you.  There is nothing that says he can't prescribe different doses day and night and breakthrough meds can also be written to be taken with the Oxy as needed.

Good luck with your doc.  I hope he starts being truthful with you.

Well said Director. I believe this has something to do with the part of the country they are in. Seems like there is a power out there where the regulations are tougher than others and makes different parts of the country tougher than others. We can all thank Drug Abusers for this.
