Alot of it is dose
Alot of it is dose dependent, as well as how you respond to the drug. Some people, when on their optimal dose, don't even realize they are taking a CNS depressant. But, I have know people that cant function by the end of the day, due to accumulation of the drug. I have had friends gain weight, get rashes, etc. A good friends father actually died from methadone, not because of an overdose, but because he had an undiagnosed cardio problem that was exacerbated by methadone. It, in some cases, can slow the QT interval of the heart, causing arrhythmias. It is a great drug for pain, but there is the potential of problems with it. If you have tried the "weaker" CII's, (oxycodone, morphine,,etc) and have not had relief, or if money is an issue, then methadone is worth a shot. The big downside with methadone though, is it has a very high binding affinity to opiod receptors. Because of this, it can somrtimes be hard to find a breakthrough med that has a high enough affinity to actually work. Switching from methadone to another med can be a problem for the same reason. It works so well, that there are cases where morphine/oxycodone don't work as well after the methadone treatment. Have you tried fentanyl? It is also a very strong med, and is similar to methadone in it's binding affinity/pain relief. It is, on average, easier to stop taking than methadone, and has fewer side-effects. Good luck, and personally, I would only try methadone if nothing else has worked, as it is pretty much a last resort med in my opinion, due to the half-life, potential toxicity, strength, and side-effects.

I have several friends that go to a methadone clinic that have had severe problems with tooth decay because of the methadone. I don't know why this is but my friends say that the dr. at the clinic told them that tooth decay is a possible side effect. By the way they take it in liquid form. I have heard 3 reasons
I have heard 3 reasons explaining the incidents of tooth decay. 1) The methadone liquid that they dispense is high in sugar, which leads to dental problems. 2) Methadone has a strong anticholinergic-effect, leading to dry mouth. The teetch need moisture to remain healthy, and excessive dry mouth can lead to decay. 3) This one I don't really agree with, but I have heard it proposed, so I figured I would include it. This theory suggests that the type of people attending a m-done clinic are addicts, therefore they are less likely to care about personal hygiene/appearance. They are basically saying these people would have tooth decay regardless of the methadone, since they are careless "junky's". Personally, I think it is probably a combination of all three. The first two assumptions probably cover the vast majority of the incidents, whereas the 3rd explanation, while maybe holding true for some, is doubtful to be the major reason for this. The combo of a daily sugary liquid, with the added dry mouth, seems to be a very good explanation for the above average rate of tooth decay amonst the population. tooth decay

I wasnt implying that you r friends don't care about appearance, I was just saying that those that don't, help to create the higher average. Have they talked to a dentist yet? I have never seen any literature that says methadone itself can lead to tooth decay when it is chronically in contact with teeth. If it was methadone itself, it would have to be from exposure, as it does not seem to be a problem in people that take the drug in pill form. Maybe the sugar used is especially detrimental, as in fructose or another "bad" sugar that the body does not tolerate well. Is it a possibility to take the dose in pill form, rather than in solution? I guess it would depend on the clinic, but if it is the sugar or methadone, I would think this would help. If it is due to anticholinergia, tablet form would not be beneficial, since the dry mouth is a systemic side-effect. This is becoming quite interesting. I will have to ask my pharmacology professor what his take on the subject is.
Dental problems with Methadone
I have been on high doses of Methadone (180 mg) for over five years now and although it's true, it causes dry mouth, I have a 32 oz cup I carry with Pepsi in it, which I sip all day. Now I know what everyone says (including my wife and dentist) that it isn't good for you, I am able to help the dry mouth. I just can't do the water thing. I have just dropped my daily dose down to 120 mg per day, my PM doc said she felt I was at too high of a dose for my age. She is lowering it by 20 mg per month. When I first started, I was drinking regular Pepsi, but have since switched to diet, with just a small amount of regular, for the flavor. This past year when I went for my check up last earlier this month, I had no cavities. I did have two with cracks that had to be replaced, but I had no new ones. When I was drinking regular Pepsi, I think I had four or five the first year. Because I had Periodontal gum disease several years ago and had sugery to correct the problem, I now have to go in every four months for a cleaning. Every other one is with my Periodontist, who does a deep cleaning every six months. I'm sure this has helped with keeping my dental problems to a minimum. I do have two other medications that also cause dry mouth, so I really do get it, hence the drink I carry.

If they are attending the clinic everyday for their dose, they cannot be abusing it because they are getting the same dose every day.... let me re-phrase that because they could abuse by telling the doc that they need alot more than they do to be comfortable(as in 60 mg a day is holding them but they take 150) but not in they way we are talkin about it consenring tooth decay.... Also, I was on the methadone clinic for almost 3 years, and my teeth are not decaying. I guess you could call me a 'junkie' because I was a heroin addict but I also never stoped personal hygine and taking care of myself(i.e. brushing my teeth). I'm not sure how much sugar the methaondone has, but it doesn't taste bad, I'm sure it has some. But anyway, I detoxed from my daily dose of 120mg to 30mg and have switched over to suboxone and hope to be finished with that soon too. please would you stop that

Fact 1: My friends go to a meth clinic. Fact 2: Since starting have had tooth decay Fact 3: Dr told them that it was a "possible" side effect. I am sure that their are a ton of other contributing factors to it though because I know of others that have not had tooth decay. 1 year of treatment: easy to taper off up to 5 years of treatment: not so easy to taper off up to 10 years of treatment: hard to taper off up to 20 years of treatment: very hard to taper off, maybe some people can't live without regular (small) doses of methadone or any other opioide. methadone detox, once the patient is addicted, is harder to taper off heroin, for example. long time consumers used to high doses could die from WD symptomes if they are "detoxed" abrupt. for side effects search this links: http://www.erowid.org/chemicals/methadone/methadone.shtml |
