methdaone maintenance + new pain management docI am on methadone for maintenance for addiction.... It helps my pain for the first 4-6 hours a day....I am about to start seeing a new pain management doctor, cause when my primary care doctor found out I was abusing what he gave me and then some... he pretty much wants to stop treating me,,, anyways I have been reffered to several pm docs till finally I found one that accepts my insurance.... other than the shots nerve blocks and discograms therepy... what do you think my options are to controll my pain I cant sleep mt leg numbing pain is horrible when in a horizontal position sleeping.. and are most pain management clinics set up to deal w/ addiction as well ...since most perscribe soo many opiates... do ya think pm doc will give methadone multiple times a day instead of me going and dosing every day. or has anyone been in this situation and have any ideas ,, ,, I am painfully...feeling addiction free..:-) Johnny ( categories: Chronic Pain and Pain Management )

whats up. im on mmt also, and i highly doubt your pm doc will give you 'done so u dont have to dose every day. its 2 different thigns, u are going to the clinic for addiction...or are you? are you going to the done clinic for pain? because since your old dr found out u were abusing, you can bet your new pm doc will know that, so he wouldnt rx u narcotics anyways. Since i am a chronic pain sufferer also, ive been rxed lyrica, which works good for the nerve pains, like the shock type pains, any pain from nerves. so u could try that. and i dont think pm clinics are set up for addiction, all they will do is not give you narcs. started like this
my back has always hurt even since my childhood. . . thru the years I have got pain meds from time to time just to help on bad days.. then I had some muscle spasms in back and neck after a car accident or two. . . just over time my back has gotten worse and worse. . also I have moved a/cs and fridges stoves dishwashers by my self up and down stairs since 1991 and I weigh 140 . . I install a/cs by myself ... I am a service manager for a 320 unit apt complex . ... well I dont do that much labor nowadays unless people are on vacation or out sick .. anyways finnaly got mri showwing buncha bad disks rubbin bone to bone in thoratic and scaitia.. they say ... my legs I gotta rub for like ten minutes after laying dawn or I cant feel my legs and I fall over...lol ....i was taking pills and pt for the past three years till the cost of extra pills after script was finished was wreking havok on my budget and my relationships,,,,, especially when my (guy) was out or i was out of money.... so I got on methadone cause I have always been addicted to one thing or another.. since as long as I can remember ..alcohol...weed..pills...coke...have always just needed something ....well now on methadone for once in my life I feel normal ........ just gotta overcome this phisical pain now... so It helps my pain for a few hours but i really am there for addiction.....and I plan on being completly honest and gonna let my pm doc know about the clinic for sure!!

I have been in the same situation as you, more or less. I was on pm until I was incarcerated for a few months. The Nurse I saw in the jail looked at my medical records from my then dr. She took it upon her self to tell my doctor that I was abusing my rx's and reported back to my doctor. The jail ultimately cut me off cold turkey and I d/t'd for about two or three weeks. I was on the Fentanyl-75mcg./hr. patch q 72h. for chronic cervical pain, Vicodin-7.5/500mg q 6 h prn for bt pain, and finally I was taking Soma-350mg. BID prn for back spasms. When I got out my dr. didn't want to see me and was chgd. doctors. I was cought abusing for my most severe pain that wasn't treated due to a years worth of tolerance and no increases and that dr. cut me off cold turkey from Methadone-10mg q 6 h for chronic pain, Norco-10/325mg 1-2 q 6 h for bt pain, and Soma-350mg qid and 2 hs(at bedtime) for spasms. I was then seeing a dr. for addiction and given Suboxone-8mg 2 in the am, then up to 2 during the day for pain. I was on this dosage for about 7months and couldn't take the pain anymore. So I decided to go to a different chronic pain doctor, but neglected to even mention the the fact of using in passed and only gave him records of my doctor that gave me all the rx's. And thank god that worked. A LITTLE WORD OF ADVICED: NEVER TELL A PM DOCTOR OF PREVIOUS NARCOTIC ADDICTIONS IF YOU ACTUALLY SUFFER FROM CHRONIC PAIN LIKE THE REST OF US AND YOU WANT HIM/HER TO HELP YOU. wow, all this sounds so farmiliar. i have also always had an addictive personallity. and was always abusing perscription drugs. then i overdosed and to make a long story short i ended up cutting off circulation to my legs for 12 hrs. so now i suffer from chronic pain in my back and legs. i take nuerontin ((very high dose)) for nerve pain. i think it works very well for me but all people are diff. i think it depends on the dr. i was perscribed methadone 10mg 3times a day until my pm dr. decided he didnt want to perscribe it to anyone anymore. now i want to change dr. but as the person above said, suboxone may be a good choice for you((as i am considering it an option for myself also)) since it helps with both pain and narcotic addiction. many people I have talked too said sub dosent work very well for cronic pain... since it has a celing the celing seems too short for the people I have talked too. but as dr lois has preached nerve pills nerotin/lycria takes away 10% pain level and sub takes away 15% pain level and with a coulpla diff things to reduce pain, tens unit, meds, therepy, it may be worth looking into tho. . . .. especialy us who just cant seem to take just one :) johnny

Pain management for addicted patients is a huge problem. Methadone for pain works best when dosed every 6-8 hours. Suboxone is another option, but you must be on 40 mg of methadone a day or less in order to switch to suboxone. I have two pain patients who have addiction disorder, on is on methadone 4x a day and the other is on 32mg a day of suboxone. methadone 4Xa day
I take methadome 60mg every morning ...it is not supposed to be for my pain, but it helps for about 6-8 hours...about what dose would I need 4x a day about 20 mg. and do you think a pain management doc would let me stop clinic to dose qid ??? I have appointment friday with new doc... it would save me 240 a month

My friend Dr. L -- WHY is pain management for people with addictions a "HUGE PROBLEM"? I mean, if they're on methadone maintenance anyway, perhaps all they need is the same drug but a different dosing schedule? Seriously, they say about 10% of folks are addicted to something, so that means there are a LOT of people with addictions who also have chronic pain. Clearly, we can't let these folks suffer, or at least, we shouldn't if at all possible. I guess what I want to know is this "problem" really a "medical problem" or is it more a problem of policy, law, and mores? In other words, if doctors could do what they do without worrying about stupid laws and technical regulations that pertain to society's pathological (IMHO) need to control what people put into their own bodies, would there still be a problem in how to treat these dual diagnosis folks, or is the problem a by-product of our society's silly need to control these things?
