I think I have a bad doctor

I am rx'd Hydrocodone/APAP 7.5/750 for pain from my chron's disease 1 every 4-6 hours prn, the standard rx. Recently I was in a car accident causing me to have short term (I hope) next pain. I saw my doctor because I was still in pain with my normal dose. He told me to just take 2 every 4-6 hours and up'd my script. I protested telling him that's way too much APAP more than double the daily limit. He told me not to worry about it. I asked him to rx a lower APAP combo or oxycodone/APAP 10/325 which I know is the about the analgesic eqv. He seemed angry and explained that he does not rx class II and Vicodin ES is the only pain med he rx's. I'm concerned about the amount of APAP. By the way this doctor is my PCP. My question is should I be worried, liver damage kidney, and should I change doctor's and report him? Let me know what you guys think.

Well he's been my doctor for about 10 years and is a good doctor but, sometimes I think I know more than he does about rx drugs. On more than one occasion he's had to look up drugs that my pych. had prescribed, clexea and ativan for example. Ativan!! I mean, that's a very widely used med esp. in hospitals.

I also have Crohn's Disease,

I also have Crohn's Disease, and my G.I. doctor would never prescribe any pain medications at all, because of the constipating effects. Also, with already having risk for severe stomach irritation, I was told to not use any drugs that contain APAP, because that could just cause more pain, and irritation. 

What does your GI doctor think about the use of drugs containing APAP?

Maybe you could switch to a Pain Management doc? You would think a PM doc would know how to treat Crohn's Disease better then the doc you are seeing now. I know you have been seeing him for 10 years but sometimes it's just time to move on. I think your health is more important. Like DyBNaps already posted, I've also heard that pain meds are not good for Crohn's Disease. If I remember correctly, it's due to inflamation so maybe a good med for inflamation is what you need? Maybe Dr. Lois could chime in and let us know what meds are best used for Crohn's?Smile

Norco 10/325 is a CIII

That would do it for me.

Norco 10/325, Lortab 10/500 and Lorcet 10/650 are all CIII and would be far better than taking two 7.5/750!

Also, any Doctor who is not worried about APAP intake would also be CANNED ASAP! Just my opinion.

 If you do not consume alcohol, you should be fine. Most likely your doctor said to take 1-2 tablets every 4-6 hours, not take 2 every 4 hours. Even if he did since your dosage is only going to be doubled like that for a short time it shouldnt be a problem. Also it is believed now that the liver can basically build up a tolerance to apap over the years. Its not like you went from taking 0mg apap to 6000mg-9000mg apap day. If he did say to make sure you take 2 every 4 hours, is it high?...yeah. But its still way under the 25g per single dose that is required to cause liver damage that will result in death death and is still  way under the 10g per single dose that is required to cause toxicity.

 When you say he got angry and said he doesnt rx CII meds, I can understand a little. If I were a doctor and someone wanted to go from Vicodin ES to the highest Oxycodone/APAP (10/325) tablet I would be thrown off guard. However he probably had already forgotten that he upped you to 15mg Hydrocodone and you are right that it is equal to 10mg Oxycodone, but he was probably thinking in his mind that you wanted to go from 7.5mg hydrocodone to 10mg Oxycodone which is a significant increase, but still isnt out of line. Its my opinion that doctors sometimes think that just cause your already on pain meds when you get, for lack of better wording, 'extra hurt' you dont need more or stronger meds. If you know of a good doctor in the area that friends/family say is more compasionate it wouldnt hurt to get a second opinion on what meds that doctor thinks you should be on, a lot of times they will want copies of your records and this can sometimes cause problems with the current doctor though. Situations like these are touchy, you could go from getting ok care to getting no care at all.

Until recently, I was taking

Until recently, I was taking 10/660 hydrocodone. My dr. was going to increase my dose from 3 times a day to 4. We talked it over and finally decided on 10/325 4X a day, but prescribing the 10/660 would have been ok too in my case since I don't have liver problems and my liver enzyme levels are normal. I just preferred the 10/325 Norco to Vicodin hp. I also might add that I have a very excellent dr.

I have to complement your doctor because he is looking up the drugs he is not familiar with when he sees you!  On the other hand refusing to write for anything other than Vicoden ES is a pretty silly way to practice medicine!  Is his practice in a low income area?  Other than that, I have nothing to add to  what Solo said.

he is not in a low income area. I don't know his reasoning for only rx Vic ES. Anyone that has chron's knows the pain that can sometimes come along with it and the complications also. I seem to have had them all. Anyone that has any experience with pain relief from chron's or other stomach diseases please let me know what works for you. My GI doctor said that demerol is rx'd to a lot of his patients. He says that it doesn't upset your stomach that much, of course there's no way my doc will rx that. I would go to a pain doc but HAP won't cover it for my condition. What a joke!!!
