I am wondering what those of you who are on extended (around the clock) opiates and also use immediate release for breakthrough pain.
Is there any benefit to being on say, Oxycontin 40mg and then Roxicodone 30mg (IR) as you are using the same drug for your pain. Has anyone perhaps tried combos?

Comments to other combos are open and welcome as well, I would like to read a broad scope of therapies.

Oxycontin and Percocet are my choices from experience and research. I am personally prescribed Oxycontin and Percocet which works very well for my pain management. Some of my family members are prescribed the same. Some are on MS Contin and Percocet for break-though. And another which is prescribed MS Contin along with Percocet and Suboxone which doesn't make much sense to me. But my family member's business med wise is personally none of my business. I just have always wondered how this particular individual is prescribed an opiate blocker with the other meds. In some ways I understand. But to some extent I just don't totally understand it. Obviously it's because I'm not this person's Dr. nor is it me to know. And like I've stated before to some extent it's from me not being a qualified physician and so much more to not understand such things in totality. Just a guy in chronic pain going by personal experience
My opinion on the best combination would be Oxycontin for the baseline med and although I would prefer roxicodone or percocet for a breakthrough med the fact of taking two oxycodone doses would make my tolerance rise too quick so I would go with Oxycontin for an ER med and then hydrocodone for a breakthrough med if it works. Depending on your amount of breakthrough pain you are experiencing you might need a stronger or weaker breakthrough med so it varies by person. For a stronger breakthrough med there are actiq or fentora (fentanyl based) or Dilaudid (hydromorphone).My first choice would be hydrocodone for a breakthrough med but it is weaker when compared to dilaudid, fentanyl, or opana.
Spade- the naxolone in suboxone has very little oral bioavailability so when taken sublingually the naxolone (opiate blocker) doesnt really have any effect at all. The bupenorphine however does bind tightly to the receptors preventing the other opiates from reaching their full analgesic effects but if a person has found a good combo why mess with it.
I agree I talked with this family member and found out the reason for it. So you'd ingest Hydrocodone with Oxycontin? Eh,Freak? Sounds like a brilliant idea. I may have to agree with you a tad on that. I'd also liked to be prescribed with my Oxycontin a Oxycodone 30mg IR. I do understand your concern for creating such a high tolerance Freak. later,Spade.
To the original poster- I have been rx'ed Oxycontin 60 mg & Roxicodone 30 mg, and this works wonders for me for pain management. (previously on OC 40mg before new dose).
has gotten me: 100mcg. duragesic q 48hrs., 4mg dillaudid, 1-3 q 4-6 hrs. for mod. BTP., 800mcg actiq q 4-6 hrs. for severe BTP. Along with 10 mg flexeril and 350mg Soma 1 @ bedtime PRN (not one of each) depending on degree of muscle spasms. Provigil to stay awake and topicals such as Voltaren Gel, Lidoderm patches, and multiple compounds. Every person is different and every disease acts differently in every person. When you find a MD that knows that and works with it, AMEN! I am still not totally pain free, but never will be. I am also on many diagnosis specific meds. but this was about pain meds. so I wanted to keep the list short.
I myself am on Morphine Sulfate Er and Roxicodone. It works very well for my pain.
In my experience the Oxycontin 40mg and roxicodone 30mg worked great i was able 2 go back 2 everything i was doing b4 the injuries, but now since my insurance ran out i am only taking the roxicodone since the generic are fairly cheap a lot cheaper then the Oxycontin anyways. I just got my dose upped to taking more roxicodone a day since i was taking off the Oxycontin
I am on Duragesic 100 every 48 hrs, Dilaudid 4mg 1-2 q 4hrs PRN,klnopin 1mg x 4 daily,Xanax PRN and Ambien for sleep. Have many DX's so I'm not going to bore you with them, just had multiple cervical surgeries and going for lumbar one soon.Right now, this combo works for me, but after time tolerance becomes an issue.Hope all works out for you, Nancie
