Inadequate treatment of pain is widespread throughout surgical wards, intensive care units, accident and emergency departments, in general practice, in the management of all forms of chronic pain including cancer pain, and in end of life care.[37][38][39][40][41][42][43][44] This neglect is extended to all ages, from neonates to the frail elderly.[45][46][47] In September 2008, the World Health Organization (WHO) estimated that approximately 80 percent of the world population has either no or insufficient access to treatment for moderate to severe pain. Every year tens of millions of people around the world, including around four million cancer patients and 0.8 million HIV/AIDS patients at the end of their lives suffer from such pain without treatment. Yet the medications to treat pain are cheap, safe, effective, generally straightforward to administer, and international law obliges countries to make adequate pain medications available.[48]

Reasons for deficiencies in pain management include cultural, societal, religious, and political attitudes, including acceptance of torture. Moreover, the biomedical model of disease, focused on pathophysiology rather than quality of life, reinforces entrenched attitudes that marginalize pain management as a priority.[49] Other reasons may have to do with health personal inadequate training, personal biases or fear of prescription drug abuse.

In the United States, Hispanic and African Americans are more likely to suffer needlessly in the hands of a physician than whites;[50][51] and women's pain is more likely to be undertreated than men's.[52] It is often recognized that a great number of patients suffering from chronic pain are being under-treated because physicians fail to provide comprehensive pain treatment. This failure may be due to physicians' fear of being accused of over-prescribing (see for instance the case of Dr William E. Hurwitz), despite the relative rarity of prosecutions (147 cases across USA in 2006), or physicians' poor understanding of the health risks attached to opioid prescription.[48][53][54] As a result of two recent cases in California though, where physicians who failed to provide adequate pain relief were successfully sued for elder abuse, the North American medical and health care communities appear to be undergoing a shift in perspective. The California Medical Board publicly reprimanded the physician in the second case; the federal Center for Medicare and Medicaid Services has declared a willingness to charge with fraud health care providers who accept payment for providing adequate pain relief while failing to do so; and clinical practice guidelines and standards are evolving into clear, unambiguous statements on acceptable pain management, so health care providers, in California at least, can no longer avoid culpability by claiming that poor or no pain relief meets community standards.[54]

Strategies currently applied for improvement in pain management include framing it as an ethical issue; promoting pain management as a legal right, providing constitutional guarantees and statutory regulations that span negligence law, criminal law, and elder abuse; defining pain management as a fundamental human right, categorizing failure to provide pain management as professional misconduct, and issuing guidelines and standards of practice by professional bodies.

Teeth Clenching during the day

Recently I have noticed my jaw has been clenched for extended periods of time during the day. I had a roommate that would clench or grind his teeth in his sleep so he wore a mouth guard, but I am doing it during the day. I have had trouble getting on a schedule. Has anyone had any experience with it. I just tried texting my old roommate about it, but he says he did it in his sleep so he wasnt sure what is causing it.

I tried searching the internet, but the only thing I could find is people clenching or grinding in their sleep. I have had trouble sleeping and I have been more stressed than usual. I got behind in school, I am transferring jobs, money is tight and I been having insomnia.

Oh an update for anyone that actually reads my posts... I wrote my primary care a letter and agreed to treat me for six more months, but he is putting me on a pain contract. I am totally fine with that, but I try to take as little of my meds as possible so and I will take the reserves on a strenuous day at work. Im not too worried about it, but it is in the back of my mind if you would quit treating me if I happened to have a little less of my medication or a little more one day if he decides to do a UA.

Thoughts, suggestions, opinions are very much appreciated.

I was browsing the internet earlier for info about Opana side effects...yes I have read the PI through and through but was curious what else might be out on the info hwy...  clicked on one site and located a junkie haven....  full of idiots that post on ways to get high and what is better... I won't reveal where it was but it made be more thankful of the day I found Pharmer....  thanks again for policing this site to make it professional for us!!!!

hows a come I cant "edit" my photo's once they have been posted?

There is one picture of the questionable Alprazolam called TRANAX-1 that I not only, post the picture, but I do a little case study.  I would like to edit it to take out all the BS n just have the image and where its from, what it is etc.

Edit button playing hide and seek

Ok mods, gentlemen can you tell me why my edit button keeps going missing all the time on my posts? I always edit several times to fix errors and such, or decide something didn't sound just right. Every so often my edit button is no where to be found.

Spell Checker for Pharmer :ieSpell for Windows 95, Windows Me, Windows NT, Windows XP, Windows 2000, Windows 98

This is a program that you can use to spell check anything you want to post on the internet, not just Pharmer.  I know there have been many inquiries about a spell checker for this site.  This one works for ANY site and it works great.  I know Windows Vista isn't listed, but that's what I run and it works perfectly.  if you have any questions about downloading or using it please don't hesitate to ask me...Ray Here is the link:

Spelling checkers keep those of us who never won a spelling bee from looking like high-school dropouts. IESpell's spelling checker for Microsoft Internet Explorer provides a safety net for interactive message boards and Web-based e-mail.

We found it unobtrusive, convenient, accurate, and fast. When you want to check your spelling, activate the program from the Tools menu, the toolbar button, or the right-click menu. The intuitive interface resembles that of any other spelling checker, with options for adding words to its dictionary and for ignoring URLs and e-mail addresses. It also provides a direct link to the online Merriam-Webster dictionary, giving you the opportunity to expand your vocabulary as you work.

Though we wish IESpell could have fixed punctuation errors and checked our spelling as we typed, we were happy it detected all our misspelled words and offered spelling suggestions. This is a great free tool for the orthographically challenged.

IESpell is a free Internet Explorer browser extension that spell-checks text-input boxes on a Web page. It should come in particularly handy for users who do a lot of Web-based text entry (Web mail, forums, blogs, diaries). Even if your Web application already includes spell-checking functionality, this utility is faster than a server-side solution. Plus, you get to store and use your personal word list across all your applications instead of having to maintain separate ones on each application.
